What is a Verification of Purchases Form?

A Verification of Purchases Form is a document used only by RSOs to submit the receipts of the items they purchased for their fiscal bills. These forms are what ORS uses to verify whether an RSO purchased enough of the requisite items to fulfill their half of a fiscal bill.
